But actually, I expect those ones are the ones we least like to see shortages in mainly because we've got such high volumes that they move through the system really quickly."

And if something fails quality control, or if there's a sudden surge in global demand,or for a variety reasons, we can see shortages. But those are unlikely related to Brexit currently."

Government is proposing that there would be fast lanes for medicines if there is disruption, say at Calais that the trucks carrying medicines could come straight through and avoid the queue and if need be, particularly sensitive medicines, so those with a short shelf life. Those that need to be temperature controlled could be flown in. But the volumes make that difficult for the majority of medicines."

"All of these are ways of mitigating disruption. The only way you avoid disruption totally is to remain in the single market and the Customs Union, because at the moment medicines might be manufactured inside the Czech Republic or Germany, they're put on a truck, the truck drives to Calais and just drives onto the next ferry and comes across. The minute you start putting customs procedures in place, that's another piece of bureaucracy, if you're having to book a particular ferry rather than just using Calais to Dover as a land bridge, if you like a roll on whenever you get there, then the truck has to get to a port on a certain time. If it misses a ferry at Dover, there's another one in half an hour, or an hour. If it misses one at Cherbourg they might be waiting six hours.

"If any of it falls outside the normal chain requirements two to eight degrees (Celsius) then it has to be destroyed, it has to be binned and actually, so even from our perspective we have to store it in fridges. So we keep it in our fridges, but of course, we can only have so much space in a fridge. And therefore, again we've got the same problem, and almost all insulin is imported. So it all comes, most of it comes from Europe, comes from elsewhere, but the vast majority comes from Europe. So actually, we are dependent on that supply chain to make sure we have sufficient stock, and from a patient's perspective, again, the same thing works. If every patient had one extra box. Well, how many extra fridges would I need just to store the stock that would require."

So we can only control what we can control and what we know, and within that we've made very robust plans. Clearly, there may be things that we haven't yet factored in, but of everything that we think we can control, then we've got robust contingencies in place."

LEADIN:
Britain's Brexit negotiations with the EU continue like a rollercoaster ride, just as there's a conclusion in sight, another chasm opens. Drug companies are among those watching closely.
Two thirds of the UK's drugs come through Europe to the southern ports of Dover and Folkstone and pharma companies are ready to roll out contingency plans to ensure there are no shortages of the UK's medicines.
STORYLINE:
They're stacked high on the shelves now, but the supply of drugs like these diabetes tablets is causing concern to patients in Britain.
Pharmacies have been told not to hoard stockpiles, but they have been told to ensure they don't run low of livesaving medicines in their store.
This chemist in a London suburb dispenses around eight thousand prescriptions a month - a thousand will be for diabetes tablets alone.
All of the top ten prescribed medicines in the UK are called generics. It means they are past their original company patents and are unbranded, this usually makes them cheaper.
The demand is a reflection of the common chronic conditions experienced by large numbers of the population throughout Europe.
Until this year Ash Soni was the President of the Royal Pharmaceutical Society as well as being a community pharmacist with chemists serving areas in south London.
He says: "The most commonly prescribed drugs are statins, which are for cholesterol, metformin, which is for diabetes, drugs which are to reduce acidity in the stomach, they tend to be the ones that we see most of, and blood pressure. But actually, I expect those ones are the ones we least like to see shortages in mainly because we've got such high volumes that they move through the system really quickly."
The government has told the pharmacists there will be enough in existing warehouse stocks to last until the end of December. It has been securing extra freight capacity and issued "reasonable worst case (RWC)" planning advice.
But this assumes there are no problems in the existing supply chain, this is not the case.
According to the Royal Pharmaceutical Society drug shortages already occur for a range of reasons. It can be a shortage of the active ingredient, or there could be a faulty batch which has to be disposed of entirely.
A recent shortage of HRT (hormone replacement therapy) has been attributed by manufacturers to a drop in the production of oestrogen, because China has relocated it's factories which produce it.
According to Soni this is the case with many other drugs.
He says: "Many of them (drugs are) manufactured in China or India, the original ingredients. So if there's a problem there in terms of the core, then there's a problem which affects us and it's not just the raw material it's about when it goes the factory and it's manufactured into the tablet or the capsule whatever it is. And if something fails quality control, or if there's a sudden surge in global demand,or for a variety reasons, we can see shortages. But those are unlikely related to Brexit currently."
But there has been concern that if there are already problems with shortages, how difficult will it be to access these drugs in the weeks and months after Brexit, regardless of whether Britain reaches a deal with Europe.
Warwick is the Director General of the Generic Manufacturers Association. These are the companies which produce the most commonly demanded, non-patented medicines in the UK.
He says his members haven't waited for the government to start contingency plans, these began as soon as the result of the referendum to leave the EU.
"Manufacturers are already buying ferry space on different routes from the EU 27 into the UK and the government is working on letting its own tenders for that sort of additional capacity.
Companies are exploring the use of less busy ports, but no-one knows how they will cope with extra freight, especially when you see the sheer volume which goes through the Channel ports of Dover and Folkstone.
Smith says: "All of these are ways of mitigating disruption.
The main concern has been about temperature controlled drugs like insulin for diabetes patients.
Soni says: "If any of it falls outside the normal chain requirements two to eight degrees (Celsius) then it has to be destroyed, it has to be binned and actually, so even from our perspective we have to store it in fridges.
Novo Nordisk  supplies half of the UK's demand for insulin.
It too says it's made contingency plans to move freight from Copenhagen where the insulin is produced, to ports like Hull in the north of England. It's also stockpiled 3.8 million packs of insulin, over four months worth of doses.
Novo Nordisk's General Manager Pinder Sahota is confident the company has enough measures in place to see their patients through any pitfalls in supply regardless of an EU deal.
After all the measures they've instigated he says: "So we're expecting now we're expecting delays of two to four days. Now, this is an unprecedented logistical operation we've never had to do this before. So we can only control what we can control and what we know, and within that we've made very robust plans. Clearly, there may be things that we haven't yet factored in, but of everything that we think we can control, then we've got robust contingencies in place."
So the most commonly used drugs in the UK aren't expected to be disproportionately in the short term, other industries though may be wondering how they will fare past the Brexit negotiations.
