Rishikesh health dept issues certificate without administering vaccine

A 52-year-old woman, Veena Valia, hailing from Uttarakhand's Rishikesh, was scheduled to get a covid vaccine on April 28. However, she was issued a certificate of vaccination without getting inoculated. Interestingly, the name of the health staff who had administered the vaccine was also registered in the directory.
