Level at 1-1 after extra time, Orlando thought they had won after Pedro Gallese saved Valentin Castellanos's penalty attempt during the shootout, which would have given them a 4-3 result.
However, Gallese had moved off his line early and referee Allen Chapman instead handed him a second yellow card and then red.
Orlando immediately tried to bring on substituted goalkeeper Brian Rowe in his place, but Chapman realised they had already used all their subs and sent Rowe back to the bench.

Finally, defender Rodrigo Schlegel stood in goal and although he wasn't able to stop Castellanos's eventual penalty attempt, he did save Gudmundur Thorarinsson's spot-kick and handed Orlando the match.
